Use this form only if you are in either grade 4 or 5 and are not experimenting with: Animals with backbones, human or animal tissue, things that could cause disease, pesticides, poisons or flammable substances, tobacco, drugs or alcohol, human beings (except those in a large group).
If the information you need for the project does not fit on this form then you will have to use the regular forms 1a, 1b, Checklist
You will need either a copy of the Adobe Acrobat Reader ® or
Microsoft Word® compatible program to read and
print these files. You can get a copy of latest version of Acrobat Reader from
ADOBE. There is no
charge for this software. If you have a copy of Adobe Acrobat Reader ®, select
the pdf form below. You will be able to edit these forms
on-line and print them. However, you will not be able to save your edits. You may receive
error messages as you open these files if you do not have the latest version of the Adobe
Acrobat Reader ®.
Some people have indicated that they have had problems printing the PDF form
provided below. One work-around is to choose
"Print" and then to check the box "Print as Image" and also
to check the box "Shrink oversize pages to paper size" and then choose
"Print".
